  • When to apply

If you are applying for transport for a new school year, make sure your application is submitted before 5 June for a September start.

Any applications received after this time may result in the assessment and assistance being delayed until after the new school year starts.

Answering ‘yes’ will not affect your eligibility for assisted travel. It helps us assess your child's level of need and means we can signpost you to other groups if necessary. Independent Travel Training is free.
  • More information about your child's travel needs

  • Additional needs

These questions help us work out your child's level of need. If you answer 'yes' to any of these, we'll ask for more information on the next page.
  • More about their additional needs

Tell us more about your child's additional needs and what makes assisted travel safe and easy for them.
  • Wheelchairs and special buggies

Tell us whether your child uses a wheelchair or buggy, or any other equipment to help them safely travel to school.
  • Wheelchair and equipment information

  • Tell us your child's height and weight

Children must use an appropriate car seat or booster until they reach either age 12 or 135cm (4ft 5in) in height. Telling us their height and weight means we can provide the correct seat for them.
  • Your ability to take your child to school

  • Parents and carers are legally responsible for making sure that their child attends school regularly. They must do everything they reasonably can to allow their child to attend school.

We only provide assistance with travel if there is otherwise no alternative for your child to attend school.

The absence of a car, other siblings, or work or study commitments will not be considered alone as a basis for travel assistance.

If you have a disability which makes it impossible for you to take your child to school, please supply medical evidence and explain how this impacts your ability to accompany your child to school.

  • If you have a condition or disability which prevents you from assisting your child to school, please provide supporting evidence of this in the upload section, at the end of this form.

  • Upload any other supporting documents you think will help us to make a decision

It's important for you to provide anything you think might be useful, as we will not seek out information on your behalf. We do not need your child's EHCP or related documents as we have access to these.
